SIGN IN BOOK
Please note that the visitor sign in book is now on a lectern just inside building entrance. All visitors are to sign in when visiting at S.Antonio. This is not only a regulation from the Department of Health but in the event of an emergency we are aware of everyone who is present in the facility.
We moved the sign in/out book from its previous location as it was rarely being signed by visitors.
To meet one of the requirements of the New Aged Care Quality Standards, will now hold case conferences with residents/representatives. This will assist in formulating the resident’s care plan. A letter went out earlier in the year with the fee statements to explain this new process. This will involve the Next of Kin and the Resident themselves.
ADVANCED CARE DIRECTIVES:
An Advance Care Directive is an important way of letting people know the residents and/or representatives about their healthcare and treatment should you find yourself in a position where they are seriously ill or injured and not able to make decisions.
Having an Advance Care Directive will make it easier for health staff if they need to make decisions for the resident. An Advance Care Directive is an important result of Advance Care Planning.
If you require further assistance or want to discuss filling out an Advanced Care Directive (see PDF link below), please speak to the Clinical Care Coordinator or Deputy Director of Nursing.
